Nutrient Spotlight: Choline

You’ve probably never heard of this nutrient before, and I wouldn’t blame you. I hadn’t either until 2016, when I joined my first research lab as an undergraduate. That lab introduced me to the field of nutrigenomics, which explores how nutrients can influence the way our genes are turned on or off. At the center of much of that work was one nutrient in particular: choline.
Choline is a bit of an oddball in nutrition. It doesn’t fit neatly into the usual categories. It’s not a vitamin, and it’s not a mineral. Our bodies can make small amounts of choline on their own, but not nearly enough to meet our physiological needs. That’s why choline is classified as an essential nutrient, meaning we must obtain it from food.
Despite flying under the radar, choline plays several critical roles in the body. It’s required to produce phosphatidylcholine and sphingomyelin, two major phospholipids that form the structural backbone of our cell membranes. Choline is also vital for brain health because it serves as the precursor to acetylcholine, a neurotransmitter involved in memory, learning, and overall nervous system function.
Beyond its structural and neurological roles, choline participates in methylation, a biochemical process that helps regulate gene expression. It also supports lipid metabolism by providing the building blocks for phosphatidylcholine used in the formation of very low-density lipoprotein (VLDL). VLDL’s job is to transport fat out of the liver. When choline intake is insufficient, the liver can’t package and export fat effectively, leading to fat accumulation AKA fatty liver disease.

Daily Intake Requirements
Unlike many other nutrients, choline doesn’t have an established Estimated Average Requirement (EAR) because the available research simply isn’t robust enough to define one. An EAR constitutes the intake level estimated to meet the needs of half the healthy individuals in a specific age and sex group. In the absence of an EAR, nutrition experts rely on an Adequate Intake (AI) value, a level thought to meet the needs of most healthy people. These Adequate Intake recommendations differ by age and life stage, reflecting how our choline needs change over time.
Men 19 years and older 550mg/day
Women 19 years and older 425mg/day
Deficiency
Most people fall short of the adequate intake, yet clinically diagnosed deficiency remains rare. This is likely because our bodies can synthesize a small amount of choline endogenously, which helps offset low dietary intake. When deficiency does occur, the consequences can be significant. Inadequate choline can impair muscle function, contribute to liver damage, and lead to non‑alcoholic fatty liver disease (NAFLD) due to the liver’s reduced ability to export fat.
Individuals who Should Pay Keen Attention to their Choline Intake
Pregnant women should ensure they consume adequate choline because it plays a crucial role in fetal brain development, particularly in processes related to memory, neural tube formation, and lifelong cognitive function.
Intensive care patients who rely entirely on tube feeding are another group at risk, because most standard enteral formulas do not include choline. Without dietary choline, these patients can quickly become susceptible to liver dysfunction, including the development of non‑alcoholic fatty liver disease (NAFLD), due to impaired fat export from the liver.
Choline may not be a headline nutrient, but its impact on health is far too important to overlook. Especially as emerging research continues to reveal just how deeply it shapes our physiology.
